Under the direct supervision of a registered nurse, performs a wide variety of patient care activities in support of Care
Coordination for Pharmacy Infusion. This includes delegated task that supports the registered nurse in : assessment,
planning, implementation, evaluation, and interaction to provide and coordinate healthcare services. Perform related duties
IV Therapy Certification
BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.
Must possess a United States driver's license
Certification in Intravenous Therapy issued by the state of California (LVN IV).
Licensed Vocational Nurse (LVN) - issued by the state of California,
Two (2) years experience as a LVN in a home health, ambulatory or acute care settings.
Excellent written and interpersonal communication skills, using tact and diplomacy in order to interact effectively with diverse cultures.
Proven skills in accurately taking and recording vital signs, weights, and heights.
Demonstrated ability to administer medications, including injections.
Demonstrated ability to collect and/or test specimens.
Proficient in Microsoft Office/technology.
- Two (2) year Case Management experience and/or Home Health
- Knowledge of case management system.
- Knowledge of quality management system.
