Under the direct supervision of a registered nurse in the Gastroenterology Care Navigation Hub, performs a wide variety of patient care activities in support of the Gastroenterology Care Navigation Hub. Responsible for answering telephone calls in the care hub as directed. Telephonic and EMR duties include but are not limited to disseminating physician results post-procedure, pending medication orders to providers, transferring symptom calls to RN triage, reviewing bowel prep instructions with patients, contacting patients with information from providers or RNs, tracking patients and ensuring care coordination across internal and external departments, contacting provider offices for records or other information and other assigned duties within the LVN scope of practice.
Additional duties are, maintaining a referral database, monitoring and working on assigned GI pools, monitoring and responding to patient mychart messages, and providing customer service assistance to the staff, physicians, and patients. Required to float and provide coverage assistance as needed and assigned to other Gastroenterology areas such as the Perlman GI clinic and the MON GI clinic. This includes prioritizing and assisting in patient triage processing, preparation and assistance with patient examinations, administering medications, drawing blood, collecting and preparing laboratory tests, and patient education. Position requires initiative, able to prioritize critical demands and the ability to work in a team environment. Provides excellence in customer service. Other related duties as assigned.
Licensed Vocational Nurse (LVN) - issued by the state of California.
BART or BLS at time of hire with commitment to get BART w/in six months of hire date.
Two years LVN experience.
Ambulatory Care Experience.
Knowledge of Gastroenterology.
Knowledge of universal precautions and proper use of body mechanics.
Knowledge and experience performing basic nursing outpatient procedures, i.e. taking vital signs, performing EKGs, administering medications, knowledge, and skill in administering various immunizations.
Knowledge and experience in outpatient care and principles of first aid, including setting up sterile trays, assisting in procedures.
