Under the supervision of the Medical Director, Center for Advanced Heart Failure and Transplantation, the Advanced Heart Failure Coordinator is responsible for the overall coordination of the Medical Director’s patients and clinical research studies. The Coordinator will facilitate new patient referrals, assess patients in the outpatient clinic, triage symptom calls, process prescription refills, respond to MyChart messages and follow up with patients regarding lab results, test results and future appointments.
The Coordinator will initiate and participate in clinical research protocols and projects, including patient recruitment and follow-up, physical assessments, lab/blood preparation/drawing, clinical/protocol documentation and collecting data for preparation of abstracts and publications. The Coordinator is expected to be self-directed, flexible and highly independent with excellent verbal and written communication skills.

Must be a graduate from an accredited school of Nursing.
RN License issued by the State of California.
BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.
Three (3) years nursing experience with a minimum of one (1) year experience in cardiology or cardiac surgery.
Proficient with Electronic Health Records and computer applications (especially Microsoft Office).
Strong written and verbal communication skills
- Bilingual in English and Spanish.
- Previous experience working in clinical research.
- Previous experience working in a cardiovascular program, particularly with advanced heart failure or transplant patients.
- Prior use of EPIC.
- Employment is subject to a criminal background check and pre-employment physical.
- Must be able to work flexible hours to accommodate clinical schedule and the scheduling of procedures- may include occasional weekend work.
- Selected candidate may be hired at the CNIII level pending review and approval by the Nurse Executive Council.
