Case Manager for a high volume Oncology Clinic physician that specializes in Breast Cancer. Clinics in North County, Encinitas and Vista. Will be responsible for assisting provider in clinic, patient education, coordinating care for scans, surgery and appointment both within the UC system and working with community services. Assisting patients and their families in coordinating patient treatment and care. Work with supporting care team and infusion staff in providing patient education, counseling, and triaging calls. Ability to work well both independently and as part of the clinical team.
Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care.

Must be a graduate from an accredited school of nursing.
RN License issued by the State of California.
BART or BLS at time of hire with commitment to get BART within six (6) months of hire date.
Minimum of six (6) months RN Nursing experience.
Excellent communication skills and customer service skills.
- Minimum of two (2) years of Hematology/Oncology experience, specifically in OB/GYN surgical oncology.
- OCN preferred or commitment to obtain within 12-18 months.
- Oncology/Breast Cancer experience
- Experience with EPIC.
- Bilingual: Spanish/English.
- Triage experience
- Must be able to work Monday through Friday, 8 hour day shift. Must be willing to go between the 2 North County clinics in Encinitas and Vista.
- Selected candidate may be hired at the CNIII or CNIV level pending review and approval by the Nurse Executive Council.
- Employment is subject to a criminal background check and pre-employment physical.
