UCSD at Home is designed to provide in-home patient care to our most complex patients living with multiple chronic conditions. We are seeking a Palliative Care physician or a Geriatrician to join the team. This is an excellent opportunity to serve a medically-diverse patient population, and to assist in growing the UCSD at Home program.
Physician will provide high quality, evidenced-based, in-home patient care to complex and high-risk patients with the primary goal and objectives of providing best-in-class care, reducing avoidable hospitalizations, and decreasing unnecessary emergency department utilization.

Conducts face-to-face medical visits at the home of the patients for routine visits, acute illness, and post-acute discharge for patients with complex medical conditions. Completes comprehensive initial and ongoing assessments of patient and family to determine palliative care needs. Clinical responsibilities include expert management of physical and emotional symptoms related to cancer or disease-directed treatment, facilitating advance care planning discussions, and collaborating closely with each patient’s primary oncology team. Completes comprehensive health assessments to identify all active and chronic conditions, as well as determine all physical, mental and social needs present at the time of the visit. Weekly participation in interdisciplinary team meetings to review active cases. The position involves home visits, full time Monday through Friday, with home-based call responsibilities that is shared. The majority of visits are conducted in person, and some may be conducted by Telehealth modalities.
Doctor of Medicine or Osteopathy
Fellowship-trained and Board-Certified or Board-eligible either in Hospice and Palliative Medicine or Geriatric Medicine
Broad expertise of services in the palliative continuum which may include outpatient, facility-based, home-based, and/or hospice care.
Current California Medical License/DEA, or eligible.
Strong ability to communicate effectively and efficiently with patients, families and team members.
Proven ability to effectively collaborate within an interdisciplinary framework.
- Employment is subject to a criminal background check and pre-employment physical.
- Must be able to work various hours, days, shifts, on-call and locations based on the 24-hour business needs.
